1. What these terms cover
These terms are about using this website. They are not about your care. If you become a patient, the paperwork you sign at that point governs treatment, consent, and payment.
2. This site is information, not medical advice
Everything here is general information about MMJ Mindful Wellness, LLC (the Practice) and about the conditions the Practice treats. It is not medical advice, and it is not a substitute for an evaluation. Please do not use it to diagnose yourself or to decide anything about your medication.
Browsing this site does not create a provider-patient relationship between us. Sending an email or requesting an appointment does not create one either.
The principal of the practice is licensed in Florida, and telehealth visits are available to people located in Florida.
3. This site is not for emergencies
In a crisis? If you are experiencing a medical or mental health emergency, call 911 or the 988 Suicide & Crisis Lifeline.
Email, voicemail, and appointment requests are not monitored around the clock, so please do not use them to reach me in an emergency.
4. No guarantee of outcomes
Psychiatric care is individual, and people respond differently to the same treatment. The Practice will bring its training and judgment to your care, but no clinician can promise a particular result. Nothing on this site should be read as promising one.
5. Links to other websites
This site links out to Zocdoc for booking, and may link to other outside resources. The Practice does not control those sites. The Practice is not responsible for their content, their availability, or how they handle your information, and their own terms and privacy policies apply once you leave this site.
6. Using this site
The text, images, and design on this site are protected by copyright and are owned by MMJ Mindful Wellness, LLC, which trades as Mindful Wellness. Please do not copy or republish them without asking first.
Please also do not use this site to do anything unlawful, or to interfere with how it works for other people.
7. Governing law
This website is operated by MMJ Mindful Wellness, LLC, and these terms are governed by the laws of the State of Florida. By using this site, you agree to these terms. Venue for any legal proceedings shall be brought exclusively in the courts of Miami-Dade County, Florida.
8. Limitation of liability and indemnification
To the fullest extent permitted by law, MMJ Mindful Wellness, LLC and its principal will not be liable for any indirect, incidental, or consequential damages arising from your use of this site or reliance on any of its content.
9. Changes and questions
If these terms change, the updated version will be posted on this page with a new date at the top.
